    • My child will attend full sessions Sons of Utah Pioneers (see address above) on the dates and times below.
    • I agree to provide transportation for my child(s) and to arrive five minutes before start time and end time.
    • My child will come to follow instructions, wear comfortable clothes, behave, participate and cooperate kindly.
    • I will Venmo $200 tuition per participant to @Annika-Cook to reserve each spot now, while space is still available. Thank you!​
    • If necessary, I will cancel my child(s) participation before June 1, 2026 or I forfeit a $50 service, camp purchases, and preparation fee.​
    • I know my child may be recorded and this may be posted, published, or sold in unlimited forms without pay.
