    • My child will attend full sessions Sons of Utah Pioneers (see address above) on the dates and times below.
    • I agree to provide transportation for my child(s) and to arrive five minutes before start time and end time.
    • My child will come to follow instructions, wear comfortable clothes, behave, participate and cooperate kindly.
    • I will Venmo a $50 NON-REFUNDABLE deposit to @Annika-Cook to reserve each spot now, while space is still available. Thank you!
    • I will also pay a remaining $150 fee for each participant now or any time before May 1, 2026 by Venmo @Annika-Cook. You may pay the full fun fee now for convenience ($200). Thank you!​
    • If necessary, I will cancel my child(s) participation before June 1, 2026 or I forfeit the remaining $150 plus, in any case, the $50 NON-REFUNDABLE camp deposit, which will be used as patronage for general camp preparation and program costs.​
    • I know my child may be recorded and this may be posted, published, or sold in unlimited forms without pay.
    • I agree that I am the legal guardian of the child named below.​
